Identify the reason for referral, diagnoses, requested services, and relevant history.
Attach records
Include demographics, insurance, recent notes, medication list, and relevant imaging or testing.
Send securely
Use the confirmed referral fax or an approved secure electronic pathway.
Clinical review
Our team reviews the referral and contacts the patient regarding appropriate next steps.
